Overview

Students applying for acceptance to Trinity Chapel Academy must provide the following information along with a completed application:

Certified copy of student’s Birth Certificate – student must be 5 years of age before September 1st to enroll in Kindergarten.
Copy of student’s Social Security Card
Georgia Certificate of Immunization – Form #3231
Georgia Certificate of Ear, Eye and Dental Exam – Form #3300
Teacher Recommendation Form – 2 forms – English and Math – are required for students applying for grades 6 – 12
Administrator/Counselor Recommendation – Grades K – 12
Release Authorization Form for students entering grades 1 – 12
Copy of student’s most recent report card and testing scores (ITBS or Stanford). TCA will use these documents to determine whether or not further testing is necessary for grade placement.

All Kindergarten applicants will be tested during the months of March and May to determine Kindergarten readiness. Written notification of acceptance or the need for re-testing will be mailed to parents the week following Spring Break.

Academic and discipline records from previous schools will be requested for all students applying for grades 1 – 12. Once records are received, the admissions committee will review the application. Academic testing will be required for all applicants. Parents will be notified in writing of their student’s testing appointment. Middle School and High School applicants will also be interviewed by the Headmaster and Middle / High School teachers. Notification of acceptance or denial will be mailed to parents as soon as possible after all steps of the application process have been completed.
